Here at Arthritis and Rheumatology Center, we value our patients. We acknowledge that going to a new place for the first time can be nerve racking. We want to do everything we can to eliminate any unnecessary stressors, which is why we include the option of printing and completing your forms at home. We think of our patients as part of our extended family, and we are excited to meet you!

Once you contact us to make your initial visit, we will send a link to your email so that you can fill out our “new patient forms” online. Forms can be also filled out at our clinic before your visit on our iPad. So we ask you kindly to be present up to 15 minutes prior to your initial appointment.
